    Week 16: The Fatigue Wall and the Iron Connection

    At 16 weeks post-op, profound fatigue hit me. Initially, I thought it was just my body fighting off an infection as my foot struggled to heal fully. But then I remembered a familiar feeling: low iron.

    Hitting the Fatigue Wall

    It has been 16 weeks since my 10-hour foot reconstruction on March 5th. Over the past few weeks, I have been feeling overwhelmingly tired. Initially, I chalked it up to the sheer metabolic toll of recovery—I assumed my body was simply exhausted from fighting off a potential infection as my lateral foot wound continues to struggle to close fully.

    But as the days dragged on and the lethargy deepened, a lightbulb went off. I recognized this specific, bone-deep exhaustion. The last time I felt this tired, it was my body telling me my iron levels had plummeted.

    How Low Iron Sabotages Slow Healing

    It is very possible—in fact, highly likely—that low iron levels are contributing to both my intense fatigue and the stubbornly slow healing of my foot. The relationship between iron, red blood cells, and wound healing is deeply interconnected.

    Here is exactly how low iron can cause delayed healing after major orthopaedic surgery:

    • Oxygen Transport: Iron is a critical component of haemoglobin, the protein inside red blood cells responsible for carrying oxygen. Healing tissue requires a massive amount of oxygen to repair cells, clear out dead tissue, and keep the wound bed healthy. If iron is low, oxygen delivery to the extremities (like my foot) is compromised, significantly slowing down the repair process.
    • Collagen Production: Iron is a required biological cofactor for your body to synthesise collagen. As we know from the Nutrition Protocol, collagen is the primary structural protein needed to rebuild skin, tendons, and connective tissue. Without enough iron, your body simply struggles to lay down this new structural framework, leaving wounds gaping.

    The VTE/Apixaban Complication

    For patients like myself on Apixaban (Eliquis) due to a history of Pulmonary Embolism, addressing iron deficiency requires care. We must ensure any iron supplementation does not cause severe gastrointestinal distress or interact poorly with our anticoagulant protocols. Always consult your haematologist or GP before starting high-dose iron.

    Can the Healing Process Cause Your Iron to Drop?

    Yes, absolutely. Going through major reconstructive surgery and dealing with a complicated, prolonged wound healing phase can actively deplete your iron stores in a few distinct ways:

    1. Anaemia of Inflammation: When your body is dealing with prolonged healing or a localized infection, it produces a hormone called hepcidin. This hormone essentially locks away your iron stores, making iron less available in your bloodstream. The body does this as a natural, evolutionary defence mechanism to starve potential bacteria of the iron they need to grow. Unfortunately, it has the side effect of making you feel incredibly tired and artificially lowering your usable iron.
    2. The Demand for Resources: Healing from a significant wound or tissue repair puts your body into a high-demand, hypermetabolic state. Your body is working overtime to produce new cells and blood vessels (angiogenesis), which requires a much higher consumption of systemic nutrients, including iron.
    3. Initial Blood Loss: Orthopaedic bone surgery is highly vascular. If the original 10-hour procedure or any subsequent wound debridements involved significant blood loss, your iron reserves may have been heavily depleted right from the start, and your body is simply struggling to catch up months later.
